The Symphony Clinic collects your name, email, phone number (if you give one), the service you ask about, how you'd like us to reply and your message to reply to you. We may also note the page you sent this from and how you found our website. We only need a phone number if you'd like us to call. We only send updates if you tick the box for that. Some of our providers, including our form service, may store information outside Australia, and we take reasonable steps to protect it.
